Job: Care Manager

Provide continuity of care, with aim for permanent placement, for children through a care management model that includes developing, expanding, accessing, and linking resources in the community to the needs of the child throughout the child's experience in the system, while documenting progress. Participates in organization's continuous quality improvement efforts.

Minimum Requiremants: Bachelors Degree in Sociology, Social Work, Psychology, Criminology, or Human Services plus one (1) year experience in providing social service, child development, family services or working in a childcare setting.
