Job Description

The Inpatient Case Manager follows the patient’s progress as they move toward the discharge process and assists with the Transition of Care. (i.e. to other care facility or home setting). The position works closely with Health Plan Case Managers to assure the patient is discharged to the appropriate level of care.

Essential Functions and Duties

  • Functions as a link between Hospitalist groups/ Hospital teams and FPG physicians for communication issues. Provide a trusted connection from their doctor’s office to help guide appropriate transition of care inside the hospital setting

  • Ensure patient and family understands discharge instructions

  • Coordinate with Complex Case Manager to ensure patient attends PCP follow up visit

  • Assist with any additional care services or DME needs

Perform case management engagement duties, such as:

  • Coordinating care with consultants while hospitalized

  • Assure patient transition of care to the next appropriate level

  • Advanced care planning

  • Long-term care patient placement

  • Family conferences to discuss plan of care

  • May cover LTAC and SNF on request

  • Other duties as assigned

Education and Requirements

  • Current State of Florida Registered Nurse

  • Case Management experience a plus

Knowledge, Skills, and Abilities

  • Ability to effectively present information in one-one and small group situations to clients and other employees of the organization

  • Excellent written/ verbal communication skills

  • Strong Organizational skills

  • Able to work with a sense of urgency

  • Strong prioritization skills
